Provider Referral

Referral Guidelines:

On Call Wound Care is committed to provide personalized care and to work and communicate with your providers as a team with a goal to heal your wound and improve your quality of life. We require a provider referral to initiate clinical services. Some of the indications to refer a patient to our services include:

  • Patients with non-healing wounds that have been present for more than 4-6 weeks.
  • Patients with non-healing wounds with significant risk factors for wound healing such as diabetes, peripheral vascular disease and renal failure.
  • Patients with chronic wounds with nerve, tendon, bone or muscle exposed.
  • Patients with non-healing venous stasis ulcers, diabetic foot ulcers, arterial ulcers, neuropathic ulcers, pressure injuries (previously called pressure ulcers), vasculitic ulcers, scleroderma ulcers and surgical or traumatic wounds

How to refer a patient

  1. Complete our referral form (download via the button below) and send via HIPPA compliant email or fax with demographic sheet with insurance information, and copies of latest progress notes with any relevant laboratory results
  2. Fax all documents to 239-257-1149 or email to beacoccaro@oncallwoundcare.com
  1. Once authorization is approved your patient will be called to schedule a new appointment. New appointments are typically scheduled for 60 minutes.
